About Manor on 1st

Manor on 1st is an assisted living community at 574 E 100 S in downtown Salt Lake City, housed in the former Salisbury Mansion, a Gilded Age residence whose historic facade was kept during a full interior renovation. The community is designed for 54 residents and held its grand opening on August 15, 2026.

The community was founded by Mark Cacciamani, MD, a Salt Lake City hospice and palliative care physician, and is physician-owned and operated. Care levels are set through a clinical assessment, and the care model includes physician-led clinical oversight, chronic disease management, and coordination between assisted living, home health and hospice.

Residents live in private suites in three sizes: Deluxe (125–199 sq ft), Premium (200–299 sq ft) and Expanded Premium (306–500 sq ft, some with an attached second room). Suites include a private bath, refrigerator, wardrobe or closet, a marble-top work space and individually controlled heating and air conditioning; some have private balconies. Residents may bring their own furniture, and adjoining suites are available.

Services include restaurant-style dining with daily specials and an always-available menu, scheduled transportation, weekly housekeeping and linen service with optional laundry, recreational and social programming, and a 24-hour emergency call system with 24-hour on-site caregivers and RN availability. The grounds include a courtyard with raised garden beds and a paved walking path. Well-trained dogs and cats are allowed.

Manor on 1st does not have a dedicated or locked memory care unit; it offers a WanderGuard system and states it will care for residents as long as it is safe to do so.

Published pricing starts at $4,300 per month for a Deluxe suite, $4,800 for a Premium suite and $6,500 for an Expanded Premium suite, plus a care-level fee of $200 to $2,000 per month. There is a one-time $3,000 community fee and a $2,000 per month second-person fee when applicable.

Advisor Insight on
Manor on 1st

A small, newly opened downtown option for families who want a 54-resident setting close to Salt Lake City hospitals, with physician-led oversight. No locked memory care, so ask how dementia progression is handled.

Best for: A small 54-resident setting in a historic downtown building, physician oversight, published pricing and pets allowed.
Consider if: No dedicated or locked memory care unit; residents who need secured dementia care would need another setting.
Ask on tour: Which suites have balconies, current care-level assessment, Medicaid acceptance, and how wandering residents are supported.
Comparison: Smaller than most downtown assisted living and set in a historic mansion rather than a purpose-built building.
