About Autumn Care Assisted Living
Autumn Care Assisted Living is a 22-bed small-residential community in Hyde Park, Utah, set against the Cache Valley landscape with the Bear River Mountains rising to the east. The address is 336 W. 300 N., a few miles north of Logan, and the small scale is the property's defining feature, most assisted-living buildings in Utah carry 50 to 100 beds, which puts Autumn Care closer in scale to a private residence than the typical senior-living building.
The care model centers on personal familiarity. With only 22 residents and a steady caregiving team, each staff member learns the history, preferences, and care needs of every person in residence. Day-to-day support covers assistance with activities of daily living, medication management, and accommodation for some memory-related conditions. The community is private-pay and Medicaid isn't accepted here, though pets are accommodated for residents.
Day-to-day life moves through shared common spaces with mountain-facing windows. Meals come out of the in-house kitchen, and the activity rhythm leans on the close-knit feel of the community, group games, music, religious services, and family events shape a calendar built around the residents in any given month. The Cache Valley location means visiting family from Logan can be there in 5 minutes. What the small scale shapes is the daily experience: it's closer to a private residence than the typical 80-to-150-bed senior building, with the staff-to-resident ratio that comes with a 22-bed building.
Tour conversations should cover staffing across all shifts, hospice partner protocols, and how the community handles residents whose care needs grow during a stay. Hyde Park sits in northern Cache County, with Logan Regional Hospital roughly 5 to 10 minutes south for medical care or emergencies. Logan's downtown shopping and dining are a similar drive south, and Utah State University is also nearby.
The community sits in a quiet residential pocket of Hyde Park with peaceful streets and easy parking for visitors. The community operates as an independent small-residential property in northern Utah. The intimate scale, the personal staff familiarity that comes with a 22-bed building, and the Cache Valley setting are the property's defining features. Families wanting a residential-care-home experience close to Logan with a tight caregiving team will find this a distinctive Cache Valley option to put on a tour list.
